Program Guidelines PTAP expects all participants to agree to abide with program guidelines. In addition, some providers of devices may require the recipient to execute a non-disclosure agreement before PTAP can deliver devices to them. Provider Guidelines (PDF) Recipient Guidelines (PDF) Enrollment Forms Both Providers and Recipients are required to complete and return an enrollment form. The purpose of these forms is twofold. First, it provides us with all of the requisite contact information. Second, by signing the form the entity indicates that it has reviewed the appropriate PTAP guidelines and agrees to abide by them.
